Rockville TMJ. More commonly referred to as "TMJ", the disorder, with its variety of symptoms, received national attention in the 1980′s as the new "dumping" diagnosis. In some social circles it was even fashionable or trendy to have TMJ.

Nevertheless, conservative estimates figure 10 million Americans have TMJ, 75 of all chronic pain disorders and may be the underlying cause of many chronic pain conditions. A Myofascial Release principle in line with the essence of Dr. Gelb’s pain theory is "find the pain, look elsewhere for the cause", as the fascial web causes many pain patterns to be referred from the primary site.

What is the Temporomandibular Joint?
Located in front of the ear canal on either side of the head, the joint connects the lower jaw (mandible) to the temporal bone of the skull. The joint allows for opening, closing and side to side movements involved in chewing, speaking and yawning. The primary muscles controlling these movements and subsequent muscular dysfunctions are the masseter, temporalis, and pterygoid muscles.

What Causes TMJ Disorders?
Known causes or contributing factors of TMJ include direct injury to the jaw, face or head from a heavyblow or whiplash. Other direct intrinsic factors include clenching or grinding the teeth, arthritis,dislocation of disc, and psycho-emotional stress. Indirect or extrinsic factors can include trauma to anywhere else in the body, poor pelvic alignment, muscle/ postural imbalances,poor oral habits and/or body mechanics at home and work. Since one TMJ cannot move independently of the other, it is considered a "bilateral joint". Being a bilateral joint makes it more prone to and directly affected by fascial misalignments. For example, well over half of all TMJ patients seen at HOPT have some form of pelvic misalignment as well.

What are the symptoms of TMJ?
Common symptoms of TMJ include:
o Pain or tenderness in the face, neck, shoulders, jaw joint
o Pain, clicking and/or popping when opening or closing the jaw
o A tired feeling in the face
o Jaw locking /difficulty opening or closing mouth
o Headaches, ringing in the ears, dizziness, ear pain, teeth pain
o muscle spasms, extremity numbness, backaches
o Difficulty sleeping, fatigue, frustration, anger, and depression

What are the treatment options for TMJ?

Things you can do to help your own symptoms:
1. Evaluate and improve stress, diet and exercise. It has been said "the best exercise is the one you do!" and therefore we encourage you to "do" something daily that has a positive impact on stress, diet and overall wellness of the body/mind. Some find it very helpful to do a few stretches throughout the day. In selecting an exercise routine good for you we suggest activities that promote good body mechanics, open and expand the body, increase oxygen and cardiovascular performance and decrease stress.

2. Use manual techniques to treat yourself.
Here is a sample self treatment protocol you can do in a few minute at home or the office: Stand with four corners of feet pressing down and toes forward, tailbone slightly under, abdomen relaxed, chest lifted, shoulders relaxed, head level, relaxed and balanced on spine. (In yoga this position is called "Mountain Pose" and when done correctly will help you feel better all over and ideally become a habit and lead to better body mechanics.)

Standing in Mountain Pose gently, rotate neck in a clockwise pain free position. Repeat in counterclockwise direction. Continue in Mountain Pose performing shoulder shrugs, shoulder rolls in forward and backward directions. Finish with pulling shoulder blades together and downward and then raising arms overhead.

Sitting with feet flat on floor, knees at 90 degrees, torso in Mountain Pose place hands on face just in front of ears and gently massage up into temples and down jaw line (spend extra time at any sore areas). Having hands along the bottom jaw line, gently push up, compressing the TMJ. Hold 1-2 minutes. Next, switch directions, gently pulling down towards your feet, distracting the jaw at the TMJ without sliding on the skin. Note that jaw position for these stretches and in general at most times should be with lips together, teeth slightly apart, lower jaw relaxed and tongue resting on the roof of the mouth.

Coping with Temporomandibular Disorders (TMD) in Teenagers

Temporomandibular disorder (TMD) is a generic term for several clinical signs and symptoms involving the masticatory muscles, the temporomandibular joint (TMJ) and associated structures.
Signs and symptoms of TMD in children and adolescents have been investigated since the beginning of the 1970s. The most frequent clinical signs of TMD are:

  • TMJ noises (on palpation by dentist);
  • Limitation of mandibular movements;
  • TMJ and muscle tenderness;
  • Headache;
  • TMJ clickings (when open or close the mouth);
  • Bruxism;
  • Limited opening of the mouth;
  • Jaw pain; and
  • Facial pain.

Clinical signs and symptoms of TMD above were present in teenagers. Muscle tenderness and joint sounds were the most prevalent, respectively. There are both local and systemic factors associated with TMD development. The forecast from various TMD signs that may become a severe disorder later in life is unclear.

The etiology of TMD has been considered to be one of the most controversial issues in clinical dentistry. Currently, TMD is regarded not an individual entity, but a group of different diseases of varying etiology and pathology. The debate still exists in this day and age because of the limited knowledge regarding its etiology and natural progression.

The role of gender in TMD is also extensively discussed in academic literature. TMD is rated to be 1.5-2 times more prevalent in women than in men. Furthermore, the predominance of women seeking treatment for TMD pain is even higher, with a ratio of 4:1. In my practice about 80% of the patients treated for this disorder are women. Moreover, the most striking gender differences have been encountered in women aged 20-40 years. It is not common for children, adolescents and the elderly to have TMD.

TMJ disorder is a complex chronic debilitating illness. Dentists have strategically developed many treatments for this condition over the years. Depend on the basic cause or causes, either one or the combination of the following treatment methods are used. They include:

  • Pain medications to ease acute pain. These, also known as the NonSteroidal Anti-inflamatory Drugs or NSAID’s, are the "first" drugs of choice. Examples of these drugs are Aspirin, Motrin and Advil.
  • Skeletal relaxer medication (for example flexeril) to downgrade muscular tension.
  • Steroid injection (hydorcortisone) to reduce inflammation.
  • Soft diet to lower the workload for the joint.
  • Apply external heat to the joint to relax the muscles around the joint.
  • Occlusal splint or Orthotic appliance to reposition the joint to its ideal arrangement.
  • Orthodontic treatment to fix teeth malalignment.
  • Physical therapy (posture training) to straighten the neck.
  • Arthroscopic surgery to remove scars inside the joint.
  • TMJ surgery to repair damages of the joints.
  • Total joint replacement surgery to replace the real joint with an artificial one.

The key words to keep in mind about TMD treatment for children are "conservative" and "reversible." Conservative treatments are as simple as possible and are used most often because most young patients do not have severe, degenerative TMD. Conservative treatments do not invade the tissues of the face, jaw or joint. Reversible treatments do not cause permanent, or irreversible, changes in the structure or position of the jaw or teeth.

Signs and symptoms of TMD clearly make an early appearance in children between the ages 12 and 18. Routine dental examination should include evaluation of these signs and symptoms to identify them. These youngsters should be watched more closely. Because most TMD problems in teenagers are temporary and do not get worse, simple treatment is all that is usually needed to relieve discomfort.

Denver and Alberta TMJ. TMJ refers to the tempromandibular joints, which are the joints of the jaw that connect the lower jaw bone (mandible) to the skull. These joints allow us to open and close our mouths, chew our food, and speak to one another (or ourselves).

A TMJ disorder is any condition which interferes with these actions. Teeth grinding, teeth and jaw clenching, and locked jaw are some examples of conditions which relate to TMJ disorders.

TMJ disorders are sometimes difficult to diagnose, as there are many different causes for these conditions. TMJ disorders are most commonly caused by muscle tension in the jaw, perhaps due to increased stress and anxiety, jaw clenching, teeth grinding, or excessive chewing. Some other habits may be linked to TMJ disorders, such as consistently sleeping on one side of the jaw, chewing a lot of gum, or overextension of the jaw.

Other causes of TMJ disorders may be misaligned jaw joints or teeth, either due to structural problems with the jaw or teeth (such as a birth defect or scar tissue), or due to an injury to the jaw or surrounding areas. Other conditions such as arthritis may also lead to TMJ disorders.

If left untreated, these disorders can lead to headaches, earaches, joint and muscle pain (especially in the face, neck, and shoulders), loss of sleep, locked jaw, difficulty opening and closing the mouth, a clicking sound in the jaw, and/or limited movement of the head and neck.

Many people experience some of these problems and, most of the time, minor problems with the TMJ will correct themselves over time. They may reappear down the road, but the condition will not normally get any worse. However, some people suffer chronically from TMJ disorders, and must receive some type of treatment to deal with the pain and discomfort they may experience.

One of the best treatment options for teeth grinding, clenching, and other TMJ disorders are the use of dental mouth guards or splints. These custom-made guards are provided by your dentist and are used to relieve tension on the jaw during sleep. They help prevent teeth grinding and, if used regularly, can help curtail these nocturnal habits. Two common types of these guards are full upper mouth guards, which fit over all of the upper teeth, or NTIs, which are worn around only the top front teeth and have a high success rate stopping clenching.

Other methods to help treat TMJ disorders are available, and the best method really depends on the exact cause/s of the disorder. If high stress or anxiety levels are to blame, then stress management techniques may be the best line of defense. If the problem is due to structural problems with the teeth or jaw, then dental adjustments may bring these problems to an end. No matter what the cause, applying soothing heat to the area is often helpful, as well. Consult your dentist to find out what steps can be taken to help treat any TMJ disorders you may be suffering from.

Bruxism, or teeth grinding, is a condition wherein the affected person clenches or grinds his/her teeth unconsciously or involuntarily, either during sleep or while awake.

People with bruxism are often not aware that this habit has developed until damage to the teeth, and other parts of the mouth, has been done. It is speculated that this habit develops due to the psychological effects of daily stress. In fact, the teeth can be so severely damaged that they become quite useless. About 5-20 percent of adults experience nocturnal teeth grinding, which is particularly problematic since it is generally not noticed until the damage has been caused to the teeth, which can take years.

What are the Symptoms of Bruxism?
The surface area of the lower and upper teeth, known as occlusal surface, is usually ground down to such an extent, that it creates an imbalance in the closure between the right and left sides of the mouth. This can lead to structural stress to the roots and tissues of the teeth and periodontal disease.

One of the effects of the teeth grinding in bruxism is tempromandibular joint syndrome, TMJ, wherein the cartilage surrounding the joints of the lower and upper jaws get irritated. This, in turn, can result in pain in the ears and jaw. Headaches due to muscle and joint strain are one of the common symptoms of bruxism.

In a nutshell, some of the symptoms of bruxism are:

  • abrasion of the teeth
  • damage caused to the tooth enamel
  • the inside part of the tooth, the dentin, being exposed
  • over-sensitivity of the teeth
  • pain in certain areas of the face
  • tense muscles of the jaw and face
  • jaw dislocation
  • headaches
  • indentations on the tongue
  • damage caused to the inside part of the cheeks
  • a clicking or popping in the temporomandibular joint

Like other sleep disorders, it is the other people that live with the person affected by bruxism who feel the brunt of the disease. This is because the teeth grinding sound can be fairly loud and thus disrupt the sleep of partners or roommates. As a matter of fact, it is usually the partner of the affected person or a member of the family who detects the condition.

Since many of the above symptoms occur in other conditions, it is best to consult a physician or dentist for an accurate diagnosis.

What are the Causes of Bruxism?
It is still not known what exactly causes bruxism, although it is thought that it occurs due to the presence of a number of factors preceding it. These are stress, oral or facial trauma, malfunction of the nervous system, and so on. It seems that certain types of personality traits may also be the root cause of teeth grinding, for example, those who are susceptible to nervous tension which cause frustration, pain, or anger. It also tends to affect people who are very competitive, have less patience, and are aggressive. What is the Treatment for Bruxism?

The treatment for teeth grinding is based on two objectives: reducing the stress that is causing bruxism, and taking care of the teeth to prevent them from being damaged.

Stress reduction can be achieved by the patient learning relaxation techniques. Activities that calm the body and the mind such as yoga and meditation can help in reducing the psychological stress which seems to exacerbate bruxism. The patient can also learn how to relax the facial muscles and jaws.

Behavioral responses leading to teeth grinding can be changed with the help of biofeedback. People affected with bruxism can learn to control the effects of their involuntary nervous system by learning how to respond properly to the changes of the conditions that affect the body.

A mouth-guard or splints can be worn at night which can help in absorbing the force of the teeth clenching or grinding, thus preventing the teeth from getting damaged.

The treatment for bruxism will be based largely on the affected person’s ability for tolerating the above methods. The trouble is, treatments like mouth-guards and splints usually cause disturbance in sleep, thus exacerbating the stress, which can worsen the condition instead of alleviating it.
